About the Role

A position that focuses on patient assessment, utilizing a comprehensive assessment to ensure a holistic care plan centered around resident's wants, needs, and goals. The ideal candidate is a nurse with excellent documentation and assessment skills, employing the nursing process and critical thinking.

Responsibilities

Coordinate the interdisciplinary assessment process for assigned residents, develop and ensure completion of Comprehensive Care Plans according to company, state, and federal guidelines, monitor residents to ensure Care Plans are followed and updated as necessary, and bring any variances to the attention of the Director of Nursing and Care Plan Team.

Requirements

Must be a Nurse with a valid state license in good standing, knowledgeable in general nursing standards and medical practices, possess excellent analytical and deductive reasoning skills, and have computer literacy and comprehensive understanding of electronic documentation programs.

Skills

Detail-oriented, employs the nursing process, uses critical thinking, coordinates interdisciplinary assessment processes, develops and ensures completion of Comprehensive Care Plans, monitors residents, brings variances to attention, and educates and leads in the post-acute care setting.
