About the Role
A position that focuses on patient assessment, utilizing a comprehensive assessment to ensure a holistic plan of care centered around the resident's wants, needs, and goals.
Responsibilities
Coordinate the interdisciplinary assessment process for assigned residents, develop and ensure completion of Comprehensive Care Plans, monitor residents to ensure Care Plans are followed, and bring any variances to the attention of the Director of Nursing and Care Plan Team.
Requirements
Must be a Nurse with a valid state license, knowledge of general nursing standards and medical practices, excellent analytical and deductive reasoning skills, and computer literacy.
Skills
Detail-oriented, employs the nursing process, uses critical thinking, and is an administrative nurse who is a leader and educator in the post-acute care setting.
