About This Role
We are looking for a detail-oriented, experienced MDS Coordinator to lead our RAI process and drive accurate PDPM reimbursement. You will work closely with nursing, therapy, dietary, and social services to ensure assessments are complete, care plans are individualized, and our facility performs at its best.
Requirements
- Current RN license strongly preferred — LPN with MDS experience considered
- Minimum 2 years of MDS coordination experience in a skilled nursing facility
- Strong working knowledge of PDPM, the RAI Manual, and iQIES
- Experience with PointClickCare, MatrixCare, or similar EHR platform
- RAC-CT certification preferred or willingness to obtain
- Excellent organizational and analytical skills
- Able to meet CMS submission deadlines consistently
