About the role
The Terraces of Phoenix, an award-winning HumanGood life-plan community, is seeking a full-time MDS Coordinator-RN. Under limited supervision, you will develop, complete, and transmit resident assessments (MDS) in accordance with current Federal and State standards. Responsibilities include performing utilization reviews, ensuring compliance with Quality Improvement and Care Planning, verifying appropriate care and length of stay for Medicare A and B recipients, and ensuring thorough documentation for billing and reimbursement.
Responsibilities
- Prepares MDS schedule in coordination with Resident Records and the Interdisciplinary Team (IDT).
- Audits completed MDS and reviews validation reports to track errors.
- Consults with Therapy to establish the most appropriate MDS ARD, consistent with PPS and OBRA regulations.
- Validates RUG classifications and ICD-10 coding through timely and accurate triple-check meetings.
- Maintains up-to-date knowledge of CMS and state regulations, including bulletin updates.
- Facilitates regularly scheduled IDT care plan meetings with residents and family members; follows up on identified issues for resident satisfaction.
- Performs quarterly reviews on each resident and updates care plans as appropriate.
- Provides in-service education for team members in accordance with OBRA and Title 22 guidelines.
- May cover for the Director of Nursing (DON) in their absence and follows up on consultant recommendations as needed.
- Participates in outside educational programs to stay current with evidence-based practices in long-term care.
- Actively engages and participates in the community’s QAPI program.
Requirements
- Completion of an accredited Licensed Registered Nursing Program (RN).
- Current Arizona RN License.
- MDS Certification.
- Minimum one-year experience as an MDS Coordinator-RN.
- Current CPR certification.
- Proficient computer skills for documentation, electronic health records, and team communication.
Bachelor of Science in Nursing (BSN) preferred.
Pay
$47 - $58 per hour, depending on experience.
Schedule
Monday - Friday, 08:00 AM - 04:00 PM.
