Responsibilities

• Conducts assessments using the MDS (Medicare & Medicaid Survey) tool
• Ensures accurate documentation of patient needs and care plans
• Collaborates with interdisciplinary teams to develop comprehensive care plans
• Monitors and updates patient care plans based on changes in patient status

Requirements

• Current NC Nurse License
• CPR Certification
• MDS knowledge and experience in long-term care preferred
