MDS Coordinator (Nurse)

AVIR Health Group is seeking a Licensed MDS Coordinator who is resident & family focused, a team builder, and excited about the opportunity to build and lead a high-performing team and culture.

About the role

The primary purpose is to complete the RAI process for each resident of the facility, as required by State and Federal regulations. This includes transmission and verification of receipt by the State.

Responsibilities

• Complete the RAI process for each resident as mandated by State and Federal regulations
• Transmit and verify receipt of RAI documentation by the State

Qualifications

• Valid Texas Nurse License (LVN or RN)
• Valid CPR Certification
• Minimum of one year of experience in a supervisory capacity in a hospital, skilled nursing facility, or other related healthcare program
• Knowledge of long-term care operational standards as set forth in the Federal Register, Conditions of Participation, and State regulations
