Responsibilities
- Coverage of Resident Assessments: Oversee admission, annual, quarterly, Medicare, and change-of-condition assessments, ensuring timely communication with nursing and other departments.
- Regulatory Compliance: Follow all Federal and State guidelines for comprehensive assessments (MDS).
- Documentation: Review MDS and care plan files for accuracy, thoroughness, and organization.
- Software Expertise: Maintain expertise in software systems to ensure correct data entry and reporting.
- Interdisciplinary Meetings: Attend and coordinate Medicaid, ITTM, and care plan meetings for smooth team communication.
- Care Plan Updates: Adjust and revise care plans as needed based on current assessments and resident status.
- Quality Reporting: Generate and distribute Quality Indicators Reports to leadership to support care improvement.
- MDS Data Submission: Ensure timely and accurate transmission of MDS assessments per reporting standards.
- Ancillary Data Oversight: Collect and report ancillary services to corporate billing for accurate processing.
Requirements
- Two years of long-term care experience.
- MDS/TILE experience.
- Fluency in MDS 3.0.
Qualifications
- Current, unencumbered, active license to practice as a Registered Nurse or Licensed Vocational Nurse in this state.
Skills
- Knowledge of Long-Term Care Medicaid, MDS, and Point Click Care.
