MDS Coordinator (LPN or RN)

About the role

The Cottage at Woodland Hills is seeking a full-time MDS Coordinator to oversee the Medicare/Medicaid/MDS resident assessment process and coordinate the development, implementation, and evaluation of the plan of care.

Responsibilities

• Coordinates the Medicare/Medicaid/MDS resident assessment process.
• Captures and implements the plan of care.
• Follows safety policies in performing nursing care.
• Coordinates and initiates as needed emergency measures according to center policy and within standards of nursing practice.
• Ability to work with OHCA and perform necessary tasks for state PASRRs in a timely manner.
• Makes sure that quality measures are monitored and overseen on a regular basis.

Requirements

• Current RN or LPN.
• Current CPR certification.
• Administrative and organizational ability and skills.
• Two years nursing experience in SNF and LTC preferred.
• Prior experience of MDS preferred.

Qualifications
