MDS Coordinator (LPN or RN)

About the role

As a MDS Coordinator (LPN or RN) at Cordova Wellness & Rehab, you'll be an essential part of our residents' care.

Responsibilities

• Attend weekly education meetings to stay updated on MDS changes.
• Coordinate the facility’s Resident Assessment Instrument (RAI) process in accordance with state and federal guidelines.
• Accurately complete all MDS assessments and any supporting assessments or clinical documentation.
• Evaluate residents' comprehensive plan of care, audit medical records for supporting documentation, and collaborate with the interdisciplinary team.
• Perform additional tasks as assigned by the Regional MDS Consultants, Administrator, and Director of Nursing.
• Maintain confidentiality of protected health information, including verbal, written, and electronic communications.

Requirements

• Active LPN or RN license for the state of Tennessee.
• 2-5 years of experience with PDPM, CMI, and Quality Measures.
• RAC Certification preferred.

Pay

$5,000 Sign On Bonus.
