Location: Yakima, WA – On‑Site Position

About the role

As the MDS Coordinator at Avalon Care Center Yakima, you will play a vital role in driving quality outcomes, ensuring assessment accuracy, and supporting exceptional resident care. This full‑time position combines clinical expertise with leadership, allowing you to make a meaningful impact on residents, families, and the facility’s success while working in a 101‑bed facility surrounded by Central Washington’s scenic outdoor recreation.

Responsibilities

• Oversee the MDS process and resident assessments.
• Ensure accurate, timely MDS completion and regulatory compliance.
• Coordinate care planning with interdisciplinary teams.
• Support Medicare and Managed Care requirements.
• Drive quality outcomes and reimbursement accuracy.
• Serve as a key clinical and operational partner within the facility.

Requirements

• Current Washington LPN license (or NLC‑eligible).
• Previous MDS Coordinator experience.
• Strong knowledge of the RAI process and case management.
• Strong attention to detail and organizational abilities.
• Ability to meet all employment screening requirements.

$46.00‑$50.00 per hour, based on experience, qualifications, and skills.
