MDS Care Plan Coordinator

About the role

River Valley Rehabilitation & Healthcare Center is seeking a compassionate and detail-oriented MDS/Care Plan Coordinator to oversee the integrity of the facility’s Medicare and Medicaid programs while ensuring residents receive high-quality, individualized care.

Responsibilities

  • Coordinate and manage MDS, PPS, and OBRA assessment schedules
  • Complete accurate and timely resident assessments and care plans
  • Monitor Medicare and Medicaid eligibility and coverage requirements
  • Conduct pre-admission nursing screenings as assigned
  • Collapse interdisciplinary Medicare meetings and care plan conferences
  • Ensure timely submission of documentation and regulatory reports
  • Collaborate with nursing staff, therapy teams, physicians, residents, and families
  • Monitor resident condition changes and update care plans as needed
  • Ensure compliance with federal, state, and facility regulations
  • Support quality assurance initiatives and reimbursement accuracy

Requirements

  • Current Colorado LPN or RN license
  • Graduate of an accredited nursing program
  • Minimum of 2 years of long-term care experience
  • Knowledge of RAI, Medicare, Medicaid, and PPS processes
  • Strong communication, organization, and leadership skills
  • Detail-oriented, dependable, and team-focused professional
  • Passionate about providing resident-centered care and clinical excellence
