About the role
River Valley Rehabilitation & Healthcare Center is seeking a compassionate and detail-oriented MDS/Care Plan Coordinator to oversee the integrity of the facility’s Medicare and Medicaid programs while ensuring residents receive high-quality, individualized care.
Responsibilities
- Coordinate and manage MDS, PPS, and OBRA assessment schedules
- Complete accurate and timely resident assessments and care plans
- Monitor Medicare and Medicaid eligibility and coverage requirements
- Conduct pre-admission nursing screenings as assigned
- Collapse interdisciplinary Medicare meetings and care plan conferences
- Ensure timely submission of documentation and regulatory reports
- Collaborate with nursing staff, therapy teams, physicians, residents, and families
- Monitor resident condition changes and update care plans as needed
- Ensure compliance with federal, state, and facility regulations
- Support quality assurance initiatives and reimbursement accuracy
Requirements
- Current Colorado LPN or RN license
- Graduate of an accredited nursing program
- Minimum of 2 years of long-term care experience
- Knowledge of RAI, Medicare, Medicaid, and PPS processes
- Strong communication, organization, and leadership skills
- Detail-oriented, dependable, and team-focused professional
- Passionate about providing resident-centered care and clinical excellence
