Requirements

• High school diploma or equivalent
• MDS Experience: 1 year (Preferred)
• RN/ LPN License

Responsibilities

• Actively participate in the development of a comprehensive, individualized plan of care for each resident.
• Ensure resident plan of care is completed, updated, and reviewed in accordance with the RAI guidelines and changes in resident care/needs.
• Assume responsibility for implementation of Clinical and Prospective Payment System Minimum Data Set completions schedule
• Direct, organize and schedule required resident assessments and related documentation for Medicare, Medicaid, managed care and Health Maintenance Organization (HMO) reimbursement
• Cover all functions of the MDS assessment process to assure compliance with federal and state requirements
• Coordinate and oversee all functions of the MDS assessment process to assure compliance with federal and state requirements
• Direct, organize and schedule required resident assessments and related documentation for Medicare, Medicaid, managed care and Health Maintenance Organization (HMO) reimbursement
• Assure compliance with federal and state requirements
• Review reports of timely completion of residents' MDS assessments and care plans and take follow up action as necessary
• Analyze Quality indicator reports for potential resident concerns and problems and review residents' care plans to be sure this information is included
• Maintain a current knowledge base regarding state and federal regulations relating to the RAI process, care planning and the prospective payment system (PPS).
