Responsibilities
- Conduct daily MDS assessments to identify resident needs and develop individualized care plans
- Collaborate with the interdisciplinary care team to provide high-quality care
- Coordinate resident data to identify trends and areas for improvement
- Develop and implement effective care plans that promote resident independence and wellness
- Communicate effectively with residents, families, and caregivers to ensure seamless care transitions
