Responsibilities
- Collect and analyze data for the Minimum Data Set (MDS) 3.0 assessment
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Ensure accurate and timely completion of MDS assessments and care plans
- Utilize clinical knowledge and expertise to identify and address resident needs
- Collaborate with the nursing team to implement care plans and achieve optimal resident outcomes
- Participate in quality improvement initiatives to enhance resident care and satisfaction
- Maintain accurate and detailed records of resident data and care plans
Requirements
- Bachelor's Degree in Nursing or related field
- Current RN license in the state of Michigan
- Minimum of 0 years of experience in a long-term care setting
- Strong clinical knowledge and analytical skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment and prioritize tasks
