About the role

This position supports the mission of the Sedgwick County Health Department by promoting the wellness of Sedgwick County residents through the Healthy Babies Program. The Community Health Worker works with high-risk pregnant and postpartum individuals to reduce infant mortality, maternal mortality, low birth weight, and prematurity.

Responsibilities

  • Community Outreach and Engagement
    • Prepare for and participate in community outreach events throughout Sedgwick County.
    • Build relationships with community organizations and referral partners to enhance service coordination.
    • Conduct outreach activities to identify and engage high-risk pregnant and postpartum individuals.
    • Manage and distribute approved client incentives.
    • Represent the Healthy Babies Program at community events while conducting MCH grant deliverables.
  • Client Support, Education, and Resource Navigation
    • Conduct screenings and assess client needs to identify barriers to healthy pregnancies and birth outcomes.
    • Provide culturally appropriate health education on maternal and child health topics, including safe sleep, substance use cessation, mental health, and other identified needs.
    • Connect clients with community resources and services through referrals.
    • Follow up with clients to ensure successful connection to services and address additional needs.
    • Aid in evaluating the effectiveness of referral and support services.
  • Documentation, Data Collection, and Program Compliance
    • Complete required screenings, assessments, and documentation according to program guidelines and grant requirements.
    • Maintain accurate, timely records and enter documentation within established timeframes.
    • Assist with data collection, surveys, needs assessments, and reporting activities.
    • Follow departmental policies, procedures, and workflows.
    • Identify opportunities for process improvement and recommend workflow enhancements.

Requirements

  • High school diploma or equivalent.
  • One (1) year in Customer Service Setting.
  • One (1) year of Community Health Worker, or Navigator experience.
  • One (1) year of experience using Microsoft Word and Excel.

Qualifications

  • Two (2) years CHW experience, with a minimum of one (1) year as certified CHW.
  • One (1) year health education or maternal and child health experience.
  • One (1) year of data entry experience.
  • Average of an Associate’s degree in business or health-related field.
  • Completion of Community Health Worker training (Certified CHW).

Skills

  • Excellent communication skills.
  • Ability to work independently and as part of a team.
  • Strong organizational and time management skills.
  • Proficiency in Microsoft Word and Excel.

Pay

$20.72 per hour.

Schedule

Monday-Friday 8:00am-5:00pm, 40 hours, Some evening and weekend hours are required.
