Masters Social Worker / MSW, Home Health

About the role

Full-time Masters Social Worker position based in the Oakdale, MN office. This is an on-site role (not remote/virtual) covering the Oakdale branch and surrounding cities: Oakdale, Woodbury, Lake Elmo, Inver Grove Heights, Cottage Grove, Maplewood, North St. Paul, and surrounding areas.

Responsibilities

• Assess patients to develop, evaluate, and revise goals and care plans
• Provide social work services in a home health setting
• Travel to all business locations within the coverage area as a licensed driver

Requirements

• One year experience as a social worker in a healthcare setting
• Master's Degree from an accredited school of social work or related field
• Valid license, registration, or permit to practice in the state of agency operation (LICSW required)
• Must possess and maintain valid CPR certification while employed in a clinical role
• Must be a licensed driver able to travel to all business locations

Pay

Salary: $72,000 – $80,000 per year, based on years of experience.

Schedule

Monday – Friday, 8:00 AM – 5:00 PM.
