The MSW reports to the Revisions Coordinator and assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes. Responsibilities
- Complete assessments on patients and caregivers throughout hospitalization and outpatient follow‑up as needed.
- Identify risk factors that patients may experience after discharge.
- Apply discharge interventions to lower the risk of an undesirable post‑discharge event.
- Bridge the discharge to outpatient provider with the patient and caregiver to address issues and questions.
Qualifications
- Master’s degree in social work or social welfare from a program accredited by the Council on Social Work Education (CSWE).
- MSW licensure by the South Carolina Board of Social Work Examiners.
- Certification in case management preferred.
Pay
Hourly pay; Pay Grade Health‑27.
Schedule
Scheduled weekly hours: 8 (PRN).
