About Matter Health

Matter Health is seeking an experienced, mission-driven Market Medical Director to provide physician leadership while delivering relationship-based primary care to older adults across an assigned market. This unique physician leadership role combines direct patient care, clinical oversight, population health, and provider mentorship.

Responsibilities

• Lead clinical care across an assigned geographic market
• Provide physician oversight and mentorship to Nurse Practitioners and interdisciplinary care teams
• Deliver primary care through a combination of in-home visits and telehealth
• Lead transitions of care for high-risk patients following hospitalizations
• Drive quality improvement initiatives and support value-based care outcomes
• Collaborate with Operations leadership to improve patient experience and market performance
• Serve as a physician ambassador for Matter Health within the communities we serve

Qualifications

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)
• Board Certified (or Board Eligible) in Family Medicine, Internal Medicine, or another Primary Care specialty
• Active, unrestricted medical license (or ability to obtain licensure within the assigned market)
• Minimum of 5 years of physician experience, preferably in primary care
• Experience leading or mentoring clinical teams preferred
• Background in value-based care, population health, or community-based healthcare preferred
• Strong communication, leadership, and relationship-building skills
• Passion for serving older adults and underserved populations
• Comfortable working independently in both field-based and virtual care settings

Preferred Qualifications

• Experience supervising or collaborating with Advanced Practice Providers
• Home-based primary care or community medicine experience
• Medicare Advantage or value-based care experience
• Population health and quality improvement experience
• Multi-site or market physician leadership experience
• Passion for mentoring providers and building collaborative care teams

• Developing and supporting high-performing Nurse Practitioners and clinical teams
