Manager Transitional Care - TGH Ambulatory Care Tranistions

Essential Functions

• Manages and directs all activities within area of responsibility.
• Incorporates TGH’s vision, mission and values in goals and programs within Transitional Care services.
• Supports AKTiVe Leadership (Authenticity, Kindness, Transparency and Vulnerability); develops trust and fosters an inclusive team member environment.
• Supports a culture of safety by promoting Zero Harm and High-Reliability.
• Prepares and delivers reports to operational leadership outlining progress toward meeting annual goals and objectives, to include performance related to finance, clinical activity, quality and safety.
• Maintains records related to operations and services that are complete, accurate, available and in compliance with all legal, regulatory and policy requirements.
• Supports the development of Transitional Care initiatives to improve quality, patient satisfaction and patient/family-centered care.
• Communicates with physicians, academic department leaders and senior leaders to maintain coordination with other TGH programs.
• Manages the complex interdepartmental and interdisciplinary relationships to assure collaboration and effective/efficient operations within Transitional Care.

Qualifications

• Bachelor's Degree - Nursing
• Florida Registered Nurse License
• Ambulatory Care Nursing Certification (AMB-BC) through American Nurses Credentialing Center (ANCC)
• Minimum five (5) years of clinical nursing experience
• One (1) to two (2) years of experience supervising or managing Transitional Care services

Pay

Minimum Salary: $90,417.60
