About the role

Manage the overall operations of the department in accordance with current federal and state regulations and guidelines. Provide operational leadership for an integrated social services program, ensuring the psychosocial, emotional, and medical needs of patients and/or a targeted population group are addressed to support optimal health and best clinical outcomes. Collaborate with the interdisciplinary team to facilitate transitions/care across the continuum, providing appropriate linkages to community resources to support care and to achieve desired clinical/treatment outcomes. Ensure cost-effective utilization of resources to support the achievement and/or improvement of dashboard measures and other key strategic initiatives.

Responsibilities

  • Oversee department operations in compliance with federal and state regulations.
  • Lead an integrated social services program addressing psychosocial, emotional, and medical needs of patients.
  • Collaborate with interdisciplinary teams to facilitate care transitions and link patients to community resources.
  • Ensure cost-effective resource utilization to improve dashboard measures and strategic initiatives.

Requirements

  • Master’s degree in Social Work.
  • LCSW (Licensed Clinical Social Worker) certification and licensure.
  • 8 years of recent relevant experience.

Skills and Knowledge

  • Working knowledge of health care delivery and case management processes in a managed care environment.
  • Knowledge of laws, regulations, and professional standards affecting clinical social work and case management (e.g., CMS, Title 22, The Joint Commission).
  • Working knowledge of health care and community resources for served populations.
  • Knowledge of quality improvement methodologies.
  • Demonstrated leadership skills, including mentoring, motivating, and engaging teams.
  • Proactive approach to identifying and resolving issues.
  • Ability to mediate and resolve differences or achieve consensus.
  • Critical thinking and analytical skills to identify trends, issues, and implement solutions.
  • Project management skills, including planning, delegation, and meeting deadlines.
  • Customer service focus in decision-making.
  • Ability to establish trust, credibility, and maintain effective working relationships across diverse groups.
  • Strong verbal and written communication skills for varied audiences.
  • Ability to make sound business decisions with minimal direction.
  • Adaptability in a dynamic, fast-paced environment with shifting priorities.
  • Integrity and ethics in handling confidential and sensitive information.
  • Proficiency in essential applications and databases related to the role.

Pay

Pay range is $161,491.20 to $242,257.60 annual salary. The compensation range may vary based on geographic location, candidate experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs.

Schedule

  • Full-time (40 hours per week).
  • Shift: Days (Monday - Friday).
  • Weekend requirements: None.
