Manager - Case Management

About the Role

Responsible for overseeing case management and discharge planning to ensure high-quality, efficient patient care that complies with regulatory standards.

Responsibilities

• Coordinate daily case management and discharge planning activities.
• Collaborate with multidisciplinary teams to optimize patient flow.
• Ensure compliance with State and Federal discharge planning regulations.
• Utilize clinical decision-making skills and medical criteria, including Interqual or Milliman guidelines.
• Monitor performance and implement process improvements.
• Work independently in a high-pressure environment, exercising critical judgment and effective communication.

Requirements

• Minimum of three (3) years of case management experience.
• At least two (2) years in a leadership role within case management.
• Graduate of an accredited Nursing program or hold a Bachelor's degree in Nursing (BSN).
• Certification in Case Management preferred.
• Knowledge of hospital information systems and performance improvement processes.
