Managed Care Coordinator II

About the role

The Managed Care Coordinator (MCC) will support Elderplan and HomeFirst, Medicare and Medicaid managed care health plans, in providing high quality, cost-effective care and services to members.

Responsibilities

• Support professional Care Management activities
• Facilitate superior care management
• Act as a liaison between members, care management team, and vendors
• Ensure compliance with departmental and CMS/DOH regulatory requirements

Requirements

• High School Diploma or equivalent
• College degree preferred
• One year prior managed care experience required
• Prior experience in a health care setting preferred
• Familiarity with utilization management/case management
