About the role
The successful candidate will be responsible for providing comprehensive patient care, managing a caseload, and collaborating with interdisciplinary teams.
Responsibilities
- Assess and document patient needs, medical history, and treatment plans.
- Provide direct patient care, including medication management and health education.
- Collaborate with physicians, nurses, and other healthcare providers to develop and implement care plans.
- Participate in interdisciplinary team meetings to discuss patient progress and plan next steps.
- Ensure compliance with regulatory standards and hospital policies.
Requirements
- Bachelor's degree in Nursing or related field.
- Current RN license in good standing.
- At least 2 years of relevant clinical experience.
- Strong communication and interpersonal skills.
- Proficiency in electronic health record systems.
Qualifications
- Experience with chronic disease management.
- Knowledge of evidence-based practice and patient-centered care.
- Ability to work independently and as part of a team.
