Responsibilities
- Assessing, monitoring, and treating wounds, skin conditions, and other dermatologic concerns
- Developing and implementing individualized treatment plans in collaboration with physicians and the interdisciplinary care team
- Maintaining and improving residents' skin integrity and overall well-being
- Making skin assessments and maintaining accurate clinical documentation
- Monitoring treatment effectiveness and recommending care plan adjustments as needed
- Providing resident and family education regarding wound prevention and treatment
- Ensuring compliance with infection control practices, regulatory standards, and facility policies
- Participating in care planning meetings and collaborating with nursing staff, physicians, therapists, and other healthcare professionals
- Assisting with admissions, discharges, rehabilitation support, and resident advocacy as needed
- Helping maintain adequate wound care supplies and treatment resources
Qualifications
- A current, active, and unrestricted California LVN license
- Wound Care Certification required
- Previous wound care and treatment nursing experience required
- A minimum of two (2) years of nursing experience
- Strong assessment, documentation, and communication skills
- The ability to work effectively as part of a multidisciplinary team
- A compassionate, resident-centered approach to care
