About the role

Committed to Caring, a qualified Licensed Vocational Nurse (LVN) administers skilled nursing care to patients on an intermittent basis in their place of residence, under the direction and supervision of a Registered Nurse, following physician orders and the plan of care.

Responsibilities

• Administers skilled nursing care to patients on an intermittent basis in their place of residence.
• Performs duties in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse.
• Develops and evaluates the plan of care in partnership with the patient, representative (if any), and caregiver(s).
• Participates in patient, caregiver, and family counseling and caregiver education.
• Communicates with all physicians involved in the plan of care and other health care practitioners as appropriate related to the current plan of care.
• Provides services that are ordered by the physician as indicated in the plan of care.
• Contributes to all phases of the QAPI program and participates in ongoing interdisciplinary assessments of patients.
• Is monitored and evaluated by the Registered Nurse (RN) in the provision of service, treatments, patient education, communication with the RN, and data collection regarding patient status and health needs as delegated by the RN.
