Requirements

• Active and unrestricted CA LVN license.
• Valid driver's license.
• Strong communication and teamwork skills.
• Ability to work independently and collaboratively.

Preferred Experience

• Behavioral health or residential experience.
• Type 1 diabetes management experience.
• Insulin administration.
• Familiarity with Dexcom continuous glucose monitoring system.
• Developmental disability services.

We are seeking a Full Time Licensed Vocational Nurse (LVN) to join our team at our program in Yuba City, CA.

Responsibilities

• Coordinating services and care in a community based program.
• Working under the supervision of an RN.
• Responsible for overall medical needs of assigned individuals in our program.
• Performing nursing duties including assessment, treatment, and follow through resolution of all health issues.
• Providing insulin and following diabetic orders.
• Charting and MAR reviews.
• Making medication reorders and coordinating with pharmacies/providers.
• Participating in community outings and appointments.
• Collaborating with families, conservators, and IDT team members.
• Supporting implementation of behavioral and medical care plans.
• Providing Type 1 diabetes management.
