- Active and unrestricted CA LVN license.
- Valid driver's license.
- Strong communication and teamwork skills.
- Ability to work independently and collaboratively.
Preferred Experience
- Behavioral health or residential experience.
- Type 1 diabetes management experience.
- Insulin administration.
- Familiarity with Dexcom continuous glucose monitoring system.
- Developmental disability services.

Responsibilities
- Coordinating services and care in a community based program.
- Working under the supervision of an RN.
- Responsible for overall medical needs of assigned individuals in our program.
- Performing nursing duties including assessment, treatment, and follow through resolution of all health issues.
- Providing insulin and following diabetic orders.
- Charting and MAR reviews.
- Making medication reorders and coordinating with pharmacies/providers.
- Participating in community outings and appointments.
- Collaborating with families, conservators, and IDT team members.
- Supporting implementation of behavioral and medical care plans.
- Providing Type 1 diabetes management.
