LVN PRN

About the role

We are seeking a Full Time Licensed Vocational Nurse (LVN) to join our team at our program in Yuba City, CA. As a MH/BH/IDD Licensed Vocational Nurse, you will coordinate services and care in a community-based program. While working under the supervision of an RN, the LVN is responsible for the overall medical needs of assigned individuals in our program.

• Perform nursing duties including assessment, treatment, and follow-through resolution of all health issues
• Provide insulin and follow diabetic orders
• Chart and MAR reviews
• Medication reorders and coordination with pharmacies/providers
• Participate in community outings and appointments
• Collaborate with families, conservators, and IDT team members
• Support implementation of behavioral and medical care plans
• Provide Type 1 diabetes management
• Schedule medical appointments

Requirements

• Active and unrestricted CA LVN license
• Valid driver's license
• Strong communication and teamwork skills
• Ability to work independently and collaboratively

Preferred Experience

• Behavioral health or residential experience
• Type 1 diabetes management experience
• Insulin administration
• Familiarity with Dexcom continuous glucose monitoring system
• Developmental disability services

Pay

Earn $33.00 - $35.00 per hour, depending on experience.
