Position Summary
As an LVN at Rock Creek Care Center, you will provide direct nursing care to residents while supervising nursing assistants during your assigned shift. You will ensure care is delivered in compliance with all federal, state, and local regulations while maintaining the highest standards of safety, dignity, and quality of care.
Key Responsibilities (Summary)
- Provide direct nursing care, including medication administration and treatments
- Supervise and support CNAs and nursing staff on your shift
- Accurately and timely chart and document care
- Communicate effectively with physicians, residents, families, and interdisciplinary teams
- Monitor resident conditions and report changes promptly
- Participate in admissions, transfers, and discharges
- Promote a safe, clean, and compliant care environment
- Support staff development, teamwork, and quality improvement initiatives
Qualifications
- Current, active LVN license in the state of California (unencumbered)
- Certified in CPR
- Graduate of an approved LVN/LPN program
- Strong teamwork, leadership, and communication skills
- Experience in long-term care preferred
- PCC experience preferred
