LVN Part-Time All Shifts

Sequoia Transitional Care is a 99-bed skilled nursing facility in Porterville, CA seeking a dedicated Licensed Vocational Nurse (LVN) Charge Nurse for a part-time NOC shift.

Responsibilities

• Provide direct nursing care to residents in accordance with individualized care plans
• Serve as the Charge Nurse during NOC shift, overseeing CNAs and ensuring high-quality resident care
• Administer medications and treatments as prescribed
• Monitor residents for changes in condition and report to RN Supervisor or physician
• Document all care provided accurately and timely in EMR system
• Communicate effectively with residents, families, and interdisciplinary team
• Maintain compliance with safety, infection control, and facility policies
• Respond to emergencies and follow established protocols
• Participate in shift handoffs, training, and care planning

Requirements

• Current California LVN license in good standing
• CPR certification
• Experience in a skilled nursing facility (SNF) or post-acute setting preferred, but not required
• Strong written and verbal communication skills
• Computer literate and comfortable with EMR systems
• Compassionate, dependable, and a team player
• Must be able to provide recent medical clearance to work in SNF
• Must pass background check and be eligible to work in the U.S.
