Kern River Transitional Care Center is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to join our nursing team on the Full-Time NOC Shift.
Responsibilities
- Provide direct nursing care to residents in accordance with physician orders, resident care plans, and facility policies.
- Administer medications and treatments accurately and timely.
- Monitor residents for changes in condition and promptly report concerns to the appropriate physician or nursing supervisor.
- Complete accurate and timely nursing documentation.
- Participate in the development and implementation of individualized resident care plans.
- Communicate effectively with residents, families, physicians, nursing staff, and members of the interdisciplinary team.
- Assist with admissions, transfers, and discharges as needed.
- Obtain and document vital signs and other clinical information.
- Ensure physician orders are carried out accurately.
- Supervise and provide direction to CNAs as assigned.
- Follow infection prevention and control procedures.
- Respond appropriately to resident emergencies and changes in condition.
- Maintain confidentiality of all resident and facility information.
- Ensure residents are treated with dignity, respect, and compassion.
- Follow all federal, state, and facility nursing regulations, policies, and procedures.
- Perform additional nursing duties as assigned.
Requirements
- Current and active California LVN license in good standing.
- Current CPR/BLS certification as required.
- Previous skilled nursing, long-term care, rehabilitation, or post-acute experience preferred.
- Strong clinical assessment and documentation skills.
- Excellent communication and organizational abilities.
- Ability to prioritize resident needs and manage multiple responsibilities.
- Ability to work effectively with residents, families, nursing staff, and other healthcare professionals.
- Dependable and committed to providing quality resident-centered care.
Pay
$28.00–$32.00 per hour, based on experience. Additional $1.00 per hour NOC shift differential.
