What You Need To Know: Under the supervision of an RN, the Licensed Vocational Nurse (LVN/LPN) provides direct patient care. The LVN/LPN observes and documents the patient's medical progress and nursing status changes under the direction of a registered nurse. This position provides direct patient care to pediatric, adolescent, adult and geriatric patients.

Responsibilities

• Performs technical nursing functions and direct patient care consistent with the established plan of care and under the supervision of an RN.
• Promotes and maintain optimal client care under the supervision of an RN.
• Maintains a safe patient environment.
• Identifies and communicates significant abnormalities in patients data to case manager and other appropriate home health team members.
• Administers prescribed medications and treatments as permitted by law.
• Affirms and reports any problems to the physician.
• Re-evaluates client’s nursing needs and reports findings to Case Manager.
• Counsels and instructs the client and family in meeting nursing and related needs.
• Prepares clinical and progress notes.
• Communicates frequently and works closely with other home care team members.
• Aids in coordinating services provided to client.
