Full-time LVN/LPN or RN (Charge Nurse) position with a double weekend schedule: Saturday and Sunday, 6AM–2PM (day shift) and 2PM–10PM (evening shift).

Responsibilities

  • Follow established standards of nursing practices and implement community programs, policies, and procedures.
  • Obtain and provide shift reports; record sufficient information for appropriate follow-up.
  • Conduct daily resident rounds, report problems to supervisor, and initiate corrective action.
  • Identify safety hazards and initiate immediate corrective action.
  • Identify resident problems and emergency situations; initiate emergency care per physicians’ orders in the absence of a physician.
  • Supervise direct care associates providing resident care.
  • Attend staff meetings, in-service classes, and committee meetings as assigned.
  • Review the Glucometer Control Log prior to blood sugar testing.
  • Follow staffing guidelines and make nursing assignments based on staff qualifications and resident population characteristics.
  • Comply with community processes for pain assessment, management, and documentation; provide treatments and deliver medications per physicians’ orders.
  • Administer and document direct resident care, medications, and treatments accurately.
  • Provide instruction for appropriate nursing care to licensed nurses and nursing assistants.
  • Document resident assessments as assigned per licensure.
  • Report untoward findings to the physician and supervisor.
  • Observe the 5 RIGHTS of medication/treatment administration.
  • Promptly report changes in residents’ conditions to the physician, Director of Nursing, and responsible party; document notifications in the clinical record.
  • Follow community admission procedures and communicate resident admission to other departments, including rehabilitation.
  • Competently perform basic nursing skills as per licensure.
  • Follow community discharge/transfer procedures and coordinate with other departments, physicians, and ancillary service providers.
  • Supervise response to residents’ calls for assistance.
  • Listen to resident, family, or physician concerns and initiate corrective action or report to the nursing supervisor.
  • Count narcotics, check emergency kits, and exchange keys each shift.
  • Monitor meal trays and nourishment to ensure proper diet is served and residents are assisted as necessary.
  • Order, receive, and store medications appropriately.
  • Observe community processes related to signing out and administering narcotics, including shift narcotic counts and documentation.
  • Notify the physician of changes in conditions, weight, medication errors, unavailable medications, abnormal lab/X-ray results, and other unusual observations.
  • Document daily notes on unstable residents, changes in resident condition, Medicare residents, and recently admitted residents.
  • Ensure the safety of residents and associates under nursing supervision.
  • Observe infection control procedures.
  • Perform all duties of a certified nursing assistant when required.

Requirements

  • Graduate of an accredited school of nursing (required)
  • Unencumbered RN or LVN/LPN license (required)
  • Proof of current CPR certification (required)
  • Minimum 1 year of experience as an RN/LVN in a clinical setting (preferred)
  • Excellent interpersonal skills
  • Reliable transportation (required)
