LVN II: Hoag Urology
Primary Duties and Responsibilities
- Assists in the assessment, planning, implementation and evaluation of patient care.
- Affords appropriate information for all patient procedures and collects and handles specimens.
- Implements and documents discharge plans for each patient.
- Recognizes ethical/legal/medical dilemmas and confers with appropriate resources for decision-making.
- Maintains a safe patient care environment.
- Supports the care and maintenance of department equipment.
- Completes and accurately charts patient records.
- Accurately records equipment and quality measures.
- Collaborates with other healthcare team members to ensure seamless care delivery.
- Ensures a safe, HIPAA-compliant environment for both patients and staff.
- Performs other duties as assigned, including phlebotomy if applicable.
Molecular Imaging & Therapy (MIT)
- Supports clinical trial record keeping and review.
- Applies demographic research criteria and reviews potential research patient charts.
- Prepares charts and updates patient profiles.
- Provides data as directed.
Interventional Radiology
- Provides preoperative instructions to ensure a safe procedure.
- Reviews patient and/or support person(s) home medication regimens via telephone.
Primary Care and Urgent Care
- Assists medical personnel with special procedures, minor surgical procedures, and/or diagnostic exams.
- Processes in-office tests, collects specimens, and performs procedures within the scope of the LVN Practice Act.
Specialty Clinics
- Performs specialty procedures such as catheters, injections, Mohs surgical patients, and injection preparation.
- Coordinates home medications, specialty pharmacies, durable medical equipment (DME), and home care.
- Manages complex care cases.
