LVN Home Health - Full Time

Responsibilities

• Provide direct patient care under RN supervision
• Follow care plans and administer treatments
• Document services accurately
• Educate patients and families
• Coordinate hospice services with the interdisciplinary team
• Manage pain and symptoms
• Participate in quality improvement activities

Requirements

• Graduate of an accredited LPN/LVN program
• Current state license
• At least three years of nursing experience (preferably in community health, hospice, or medical/surgical settings)
• Good communication and organizational skills
• CPR certification
• Valid driver’s license with insured vehicle
• Understanding of hospice care needs

Pay

Compensation is per visit.
