LVN : Home Health

Responsibilities

• Provides care within the scope of the Vocational Nurse Practice Act and regulatory standards under the direction of a registered nurse or physician.
• Assists in the assessment, planning, implementation, and evaluation of patient care.
• Follows patient identification procedures, provides information for patient procedures, and collects and handles specimens.
• Assesses and implements prescribed interventions for patients and documents discharge plans.
• Recognizes ethical/legal/medical dilemmas and consults appropriate resource personnel for decision-making.
• Participates in maintaining a safe patient care environment and assists with the care and maintenance of department equipment.
• Ensures complete and accurate charting of patient records and record-keeping of equipment and quality measures.
• Collaborates with other healthcare disciplines to ensure timely, safe, and quality care delivery.
• Provides a safe, HIPAA-compliant environment for patients and staff.
• Performs other duties as assigned.

Molecular Imaging & Therapy (MIT)

• Supports clinical trial record-keeping and review.
• Applies demographic research criteria and reviews potential research patient charts using inclusion/exclusion criteria.
• Prepares charts, updates patient profiles, and provides data as directed.
• Performs front office functions.

Interventional Radiology

• Assists in providing preoperative instructions to patients and/or support persons to ensure a safe procedure.
• Reviews patient questions or concerns to alleviate anxiety before appointments.
• Obtains critical patient history and home medication regimens via telephone in preparation for procedures.

Primary Care and Urgent Care

• Assists medical personnel with special procedures, minor surgical procedures, and/or diagnostic exams.
• Processes in-office tests, collects specimens, and performs procedures within the scope of the Vocational Nurse Practice Act.

Home Health/Hospice Float

• Provides care in both home health and hospice settings.
• Assignments may vary daily and cover territories throughout Orange County.

Requirements

• High School diploma or equivalent.
• One or more years of experience in a healthcare facility (Specialty Clinics may hire new grad LVNs with no experience).
• Basic keyboarding skills and knowledge of Microsoft Office Suite (Outlook, Word, Excel) preferred.
• Basic knowledge of medical terminology preferred.
• Experience with Electronic Medical Records preferred.

Home Health

• At least one (1) year of experience in public health or home care nursing.
• Must be a licensed driver with an insured and well-maintained automobile.

Hospice

• Community health/hospice or medical/surgical experience preferred.
• Understands the philosophy of hospice and the needs of the terminally ill.
• Must be a licensed driver with an insured and well-maintained automobile.

Primary Care and Urgent Care

• Completion of New Hire LVN course – Primary Care or Urgent Care within 6 months of hire.

Qualifications

• Current LVN License to practice in the State of California.
• Current BLS Certification.
• Phlebotomy Certification preferred.
