About the role
The LTSS Service Coordinator- RN Clinician is responsible for managing member cases, providing supervision to non-RN clinicians, and developing, monitoring, evaluating, and revising care plans to meet member needs.
Responsibilities
- Identifies members for high-risk complications and coordinates care.
- Manages members with chronic illnesses, co-morbidities, and/or disabilities to ensure cost-effective and efficient utilization of health benefits.
- Develops, monitors, evaluates, and revises the member's care plan to meet the member's needs.
- Performs face-to-face clinical assessments for the identification, evaluation, coordination, and management of member's needs.
- Coordinates services for the member by assigning appropriate tasks to non-RN clinicians, verifying and interpreting member information, conducting additional assessments, and reviewing and providing input on the non-RN clinicians' performance.
- Interfaces with Medical Directors, Physician Advisors, and/or Inter-Disciplinary Teams on the development of care management treatment plans.
- Directs and/or supervises the work of any LPN/LVN, LSW, LCSW, LMSW, and other licensed professionals other than an RN.
Requirements
- Requires an RN and minimum of 3 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role.
- Current, unrestricted RN license in applicable state(s) required.
- May require state-specified certification based on state law and/or contract.
Qualifications
- MA/MS in Health/Nursing preferred.
- Travels to worksite and other locations as necessary.
Skills
- Ability to identify and manage high-risk members.
- Experience in chronic illness management.
- Knowledge of care management and coordination.
- Ability to develop and implement care plans.
- Interpersonal skills to interface with various healthcare professionals.

Pay
The salary range for this specific position is $41.84/hr to $62.76/hr. Actual compensation may vary based on location, work experience, education, and skill level.
Schedule
Location(s): New Jersey. Associates are required to work at an Elevance Health location at least once per week, and potentially several times per week.
