LTSS Service Coordinator-RN Clinician

About the role

The LTSS Service Coordinator-RN Clinician is responsible for managing members' cases, providing supervision to non-RN clinicians, developing, monitoring, evaluating, and revising care plans, and ensuring member access to services.

Responsibilities

• Identifies members for high-risk complications and coordinates care.
• Manages members with chronic illnesses, co-morbidities, and disabilities to optimize health care costs and efficiency.
• Develops and monitors care plans, establishes short and long-term goals, and identifies members for alternative care or waiver programs.
• Directs and supervises non-RN clinicians in coordinating services, assigns tasks, verifies information, conducts assessments, and reviews performance.
• Interfaces with Medical Directors, Physician Advisors, and Inter-Disciplinary Teams to develop care management treatment plans.
• Assists with problem-solving related to providers, claims, and service issues.

Requirements

• Requires an RN license in applicable state(s) and at least 3 years of experience in a Service Coordinator, Case Management, or similar role.
• May require state-specific certifications based on state law and/or contract.

Preferred Skills, Knowledge, and Experience

• MA/MS in Health/Nursing preferred.

Schedule

Associates are required to work at an Elevance Health location at least once per week, and potentially several times per week, depending on the role and location.
