LTC Transportation Specialist

Duties

• Works with management to provide transportation to residents based on transportation schedule.
• Provides, completes, and documents resident care as assigned in a timely and accurate manner.
• Attends and participates in facility in-service programs about transportation needs.
• Reports any change of condition to charge nurse.
• Moves residents about the facility.
• Moves residents in and out of hospitals and healthcare facilities.
• Manages transportation schedule.
• Communicates key information to facility team regarding transportation.
