LTC Primary Care NP - PT - Waynesboro, TN

About the role

Responsibilities

• Develop and implement personalized treatment plans based on patient needs
• Offer medication management, including prescribing and monitoring medications
• Collaborate with a multidisciplinary team of healthcare professionals to ensure patient care
• Maintain accurate medical documentation in line with regulatory standards
• Provide education and support to patients and their families regarding mental health conditions and treatments

Qualifications

• Current Nurse Practitioner license in specified location
• At least two years of experience is required
• Experience providing care to geriatric populations in long-term care settings is preferred but not required
• Excellent communication skills, especially when working with facility staff and residents' families
• Ability to manage time effectively, including flexibility in scheduling and responsiveness to patient needs
