Details
- Clients throughout Westchester requiring shifts from 8hrs to 12hrs
- Day and Night Shifts Available
- Work with us full-time or around your current work schedule
- Shifts are assigned to provide 1:1 care in the home environment
Qualifications
- NYS LPN license
- BLS certification
- Minimum of One (1) year demonstrated experience as an RN
- Ability to put patients at ease and explain physician's instructions
- Ability to multi-task with excellent organization skills
- Knowledge of medical terminology
- Team player and ability to work independently
- Valid driver's license or NYS Non-Driver photo ID card (required)
- Drivers preferred
Responsibilities
- Assess needs of the clients to develop a plan of care
- Consult with Management on complex/unique cases
- Provide a plan of care with support of the Clinical/Medical/appropriate multidisciplinary members to ensure the services follow the plan of care by assessment, planning, coordination, counseling, teaching, and supervision to meet needs of the clients/patients
- Monitor/Assess patients/clients' condition and effects of the care plan
- Oversee/supervise the care provided to the patient by Licensed Practical Nurses and Paraprofessionals
- Manage, plan, coordinate, and execute the patient's safe discharge from service
- Ensure that physician's orders, discharge summary, and communication to team members, patient, and family members adhere to company and regulatory requirements
- Communicate and consult with patient's MD with all matters when needed, manage and coordinate all aspects of assigned caseload
- Assist patients with activities of daily living
- Document content and process of all services provided in the internal system properly
- Perform other related duties and participate in special projects, as assigned
