LPN Wellness Nurse for Memory Care 3-11

About the role

This is a full-time position with a 3‑11pm shift, including every other Saturday and Sunday, in our Memory Care Community.

Responsibilities

• Provide nursing care in accordance with the client's plan of care, including comprehensive health and psychosocial evaluation.
• Monitor the client’s condition.
• Coordinate health promotion, prevention, and services.

Requirements

• Two years of LPN experience required.
• Two years of experience in assisted living or a nursing home.
• Licensed Practical Nurse license in good standing.
• Ability to ensure the community follows all federal, state, and local laws and regulations pertaining to resident services and care.
• Demonstrate good judgment, problem‑solving, and decision‑making skills.
• Effective organization, time management, and written and verbal communication skills.
• Ability to handle multiple priorities and delegate assignments.
• A dedication to seniors and their well‑being is essential.
• Ability to work nights and weekends.
