About the role
Acts as a liaison between clinical staff and management, focusing on coordinating the clinical operations of the practice, panel management, patient education, and health coaching. In partnership with the practice leadership team—including clinicians, CBHM, MSW, RN, and practice manager—supports care management efforts through process improvement, workflow redesign, training assistance, and delegation. Coordinates with the lead Medical Assistant to provide oversight for clinical operations.
Responsibilities
- Collaborates with practice leadership to conduct daily huddles and weekly care team meetings, reviewing practice population health data to improve care and achieve practice goals.
- Supports the delivery of patient care by:
- Preparing patients for exams by obtaining and documenting vital signs and chief complaints.
- Preparing exam rooms based on chief complaints.
- Providing general medical treatment such as administering medications, applying appliances, dressings, and performing first aid for injuries or lacerations as ordered by a physician.
- Performing phlebotomy and obtaining specimens for processing.
- Administering vaccines and completing required documentation.
- Performing diagnostic tests such as EKG and pulmonary function tests as ordered by the physician.
- Collaborates with the practice team to support population health efforts, including panel management using value-based clinical registries and coordinating test and referral tracking.
- Supports care management efforts by:
- Leading patient education and training Medical Assistants in delivering education to patients.
- Leading self-management efforts for patients with chronic conditions, including training Medical Assistants in health coaching and motivational interviewing.
- Coordinates transitions of care under the direction of clinicians by:
- Ensuring outreach to patients within 48 hours of hospital discharge or within 7 days of ED visits, scheduling follow-up visits as appropriate.
- Facilitating hospital and other records into the EMR prior to visits.
- Managing Coumadin and patient follow-up.
- Collaborates with the lead Medical Assistant to maintain par levels of clinical supplies and performs daily regulatory testing to ensure clinical equipment accuracy and compliance.
Requirements
- Minimum of 6 months of experience in a physician practice; primary care experience preferred.
- Knowledge of minor surgical procedures and sterile technique.
- Graduate of an approved LPN School of Nursing.
- Current NJ LPN License.
- Current CPR certification.
Pay
$24.51 - $36.89 hourly.
