Monday – Friday 8:30 am - 5:00 pm in Stamford

About the role

The Licensed Practical Nurse (LPN) coordinates patient care following all established standards of practice within the State and Federal guidelines. Provides staff mentorship. Serves as a resource to both staff and management in problem identification and resolution.

Responsibilities

  • Reviews schedules and organizes patient flow;
  • greet patients, documents patients concerns, questions etc…;
  • accompanies patient to exam/procedure room and prepares patients for physical exam;
  • Initial patient interview;
  • reviewing patient medical profile, and documents all required charting in EMR/manual charting as appropriate;
  • Obtains and accurately documents patient vital signs, weight: reports any abnormalities to physician;
  • Assists with specimen collection and testing as necessary. Documents pertinent information;
  • Provides minor treatment as directed by physician, assists physicians with minor procedures;
  • Provides education as indicated after evaluating the learning needs of the patient and or family;
  • As required, schedules patient testing and pre certifies if insurances requires;
  • Responds to telephone messages (patient, physician offices etc…);
  • End of month checks for expired supplies and medication;
  • Stocks rooms/maintains inventory or exam/procedure rooms;
  • Orders supplies as needed;
  • Exam/procedure room & equipment cleaned after each patient;
  • Prepares exam /procedure room for next patient & physician needs;
  • Notifies patients of test results (per physician instruction) and follow up;

Qualifications/Requirements

  • Graduate of an accredited school of nursing;
  • Valid license as Licensed Practical Nurse in CT;
  • BLS Certification;
  • Excellent customer service / communication skills;
  • Knowledge of medical terminology;
  • Minimum of 2 years previous experience in a similar position preferred;
  • Must be able to read, write and speak the English language.
