About the role
Licensed Practical Nurse (LPN) providing skilled nursing care in home settings, primarily managing pediatric trach/vent patients.
Responsibilities
- Assess, plan, implement, and evaluate patient care using the nursing process.
- Administer prescribed nursing services and interventions.
- Monitor signs and symptoms to identify physiological and psychosocial changes.
- Collect, analyze, and document patient data and nursing diagnoses.
Qualifications
- Current LPN/LVN license in the practicing state.
- Valid BLS certification and current TB or Chest X-Ray.
- Preferred: at least one year of professional nursing experience.
Skills
- Strong assessment and documentation skills.
- Ability to provide patient-centered care.
- Adherence to prescribed orders in a home environment.
