Summary of Position
The LPN plays a vital role in coordinating care for individuals within designated residential settings. Working collaboratively with interdisciplinary team members and house managers, the LPN ensures adherence to healthcare regulations and standards, fostering a safe, supportive environment that promotes the health, dignity, and overall well-being of those served.
Coordination of Services
- Residential-Based Nursing Support: Maintain a consistent and collaborative nursing presence within residential homes, focusing on direct engagement with individuals served and supporting staff in real-time care coordination.
- Health Assessments: Conduct wellness and illness evaluations following urgent care visits, hospital visits, falls, or other health-related events, ensuring thorough documentation and follow-up of health needs.
- Documentation: Complete timely and accurate documentation of assessments and communications within each residence, ensuring continuity and transparency in care delivery.
- Maintain Accurate Medical Records: Assist with reviewing and updating medical and medication records, including healthcare provider orders, medication lists, immunization records, healthcare provider visits, rehabilitation and other healthcare related documentation.
- Medical Appointments: Accompany individuals to physician’s appointments as needed, supporting communication and advocacy for optimal health outcomes.
- Advocate through Communication: Work closely with healthcare providers, including visiting nurses, laboratories, hospice care, pharmacies and physician offices, to support individualized plans of care.
- Collaborate on Treatment Plans: Coordinate care while in the residence and/or program, working closely with team members and individuals served to ensure treatment plans are responsive and person-centered. Work with health care professionals and staff to formulate, implement and monitor medical treatment plans.
- Support Nutritional Needs: Provide dietary support with preparation of specialized diets and eating. Monitor nutrition plan compliance and assess changing needs.
- Facilitate Transitions of Care: Assist with new admissions, and assist with coordinating admissions and discharges from healthcare facilities, including hospitals, rehabilitation centers, ensuring smooth handoffs.
- Quality Compliance: Adhere to medical policies and procedures to ensure compliance with state, federal, and agency regulations.
- Administer treatments and medications, following the Department of Public Health’s (DPH) Medication Administration Program (MAP) policies and prescriber orders.
- Implement and review health care treatments, prescriber orders and protocols for accuracy, ensuring compliance with funder regulations and ease of use and understanding for program staff.
- Participate and schedule programmatic reviews/ audits, including medication audits and other quality improvement initiatives. Update information as needed to maintain record compliance.
- Execute duties to reflect reasonable safety, infection control standards and privacy standards. Universal/standard precautions must be utilized and training obtained in areas that constitute risk.
Knowledge and Experience Required
- Massachusetts Licensed Practical Nurse.
- Valid license as an LPN in the Massachusetts Board of Registration in Nursing. Licensure must remain active and in good standing with the Massachusetts Board of Registration in Nursing.
- Three years of nursing experience as an LPN preferred.
- Must possess a valid driver’s license.
Skills and Abilities Required
- Performs duties involving an intensive knowledge of nursing care and related policies and procedures and the application to situations not previously covered.
- Requires the use of judgment in the modification or adaptation of procedures to meet unusual situations.
- Work with confidential data such as individual case records which if disclosed inadvertently could have significant internal effect or minor external effect.
- Proficient computer skills including Word, Excel, and other related MS Office applications.
- Comeptent and comfortable with learning new computer systems, programs and applications.
