Responsibilities
- Provide compassionate, one-on-one nursing care in the comfort of patients' homes
- Administer and manage medications
- Monitor and document patient conditions
- Perform treatments as ordered by the physician
- Educate, encourage, and support family members
- Work alongside the Phoenix clinical team to provide exceptional care
Requirements
- Active Illinois RN or LPN license (or Compact license)
- Strong assessment and communication skills
- A compassionate heart, dependable character, and self-motivated spirit
- Valid driver's license and reliable transportation
