About the Role
We're looking for a highly motivated and organized LPN/RN MDS Coordinator to coordinate the resident assessment process, identify individualized care needs, and develop individualized care plans. As a key member of our care team, you'll work closely with our licensed nurses, therapists, and other healthcare professionals to ensure the delivery of high-quality care to our residents. Responsibilities
- Coordinate the resident assessment process, including scheduling and conducting MDS assessments, and developing individualized care plans
- Identify individualized care needs and communicate with the interdisciplinary care team to develop and implement comprehensive care plans
- Collaborate with licensed nurses, therapists, and other healthcare professionals to ensure the delivery of high-quality care to our residents
- Maintain accurate and up-to-date records of resident care, including assessment results, care plans, and progress notes
- Participate in facility-wide quality improvement initiatives to promote resident-centered care and improve resident outcomes
- Provide education and support to family members and caregivers on resident care and services
Requirements
- Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in the state of Missouri
- Experience in long-term care or related field preferred
- Strong organizational and communication skills
- Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines
- Strong knowledge of MDS and RAI processes
