LPN/RN - Full-Time 6am-6:30pm

About the role

The LPN plays a crucial role in providing quality patient-centered, evidenced-based care to our residents. They are responsible for utilizing the principles of Relationship Based Care and performing duties such as initial data collection and participating in the implementation of person-driven care plans.

Responsibilities

• Utilize principles of Relationship Based Care
• Perform duties related to initial data collection on all residents
• Participate in the implementation of person-driven care plans

Requirements

• Current licensure as a Practical Nurse in good standing
• Ability to read, write, and communicate in English
• Clinical experience preferred
