LPN / RN Care Manager

About the role

The LPN Medication Nurse provides licensed nursing support in senior living settings, focusing on safe medication administration, health monitoring, and accurate clinical documentation.

Responsibilities

• Administer medications and treatments according to physician orders, following the Six Rights of Medication Administration.
• Document all procedures accurately in MAR/TAR in real time.
• Monitor residents' symptoms, administer medications for relief, and report any refusals or adverse reactions promptly.
• Perform Service & Health Updates (SHUs), transcribe physician orders, and support care plan updates.
• Observe and report physical, emotional, or cognitive changes in residents.
• Maintain medication carts, complete narcotic counts, and ensure compliance with regulations.

Qualifications

• Active LPN license in good standing.
• CPR and First Aid certification.
• Strong assessment, documentation, and observation skills.
• Experience in senior care or medication administration preferred.

The role is vital in preventing medication errors, early detection of health changes, and safeguarding resident safety through clinical precision and regulatory adherence.
