LPN Response Nurse | Assisted Living | Full time

Brief Description

Holland Home, Michigan’s largest non-profit senior care provider, is seeking a compassionate and skilled LPN Response Nurse to join their Raybrook Assisted Living team.

Position Details

• Location: 2121 Raybrook St SE, Grand Rapids, MI 49546
• Schedule: 36 hours per week, 6:30pm - 7:00am
• Week 1: Tue, Fri, Sat
• Week 2: Sun, Wed, Thu

Qualifications

• Current MI LPN license
• CPR certification required
• Experience with geriatrics preferred
• Basic computer skills for charting in PCC
• Strong interpersonal and leadership skills

What You’ll Do

• Provide direct nursing care, including medication administration, assessments, and treatments
• Respond to emergencies and initiate appropriate care
• Supervise and coach care staff, ensuring compliance with standards and regulations
• Build trusting relationships with residents and families, offering emotional support
• Act as a liaison between residents and physicians, managing orders and communication
• Maintain accurate documentation and participate in interdisciplinary care planning
